Summary Plan Description (SPD)

The SPD summarizes the key provisions of the Plan and includes important information about your benefits from the Plan.

Summary of Material Modifications (SMM)

The SPD is updated from time-to-time and includes additional information and changes to the Plan in the form of an SMM.

Annual Funding Notice

The Annual Funding Notice provides basic financial information regarding the Plan and informs you of the funding of the pension plan.

FORMS

Distribution Application

To apply for your pension, you will need to complete a Distribution Application. Please complete the forms and mail along with any noted documentation to the Fund Office.

Direct Deposit Form

To sign up for direct deposit complete a Direct Deposit Form and return it to the Fund Office.

Tax Withholding Form

To change the amount of the taxes withheld from your pension payment, complete a Tax Withholding Form and return it to the Fund Office.

Change of Address Form

Complete this form to change or correct your mailing address and return it to the Fund Office.

Change of Name Form

Complete this form to change or correct your name and return it to the Fund Office.

Beneficiary Designation Form

To designate a beneficiary for your death benefit, you must fill out this form and return it to the Fund Office.

FREQUENTLY ASKED QUESTIONS

There is a two-step process to apply for your pension, you will first need to complete a Distribution Application. You should apply for your pension at least 90 days prior to your retirement date. Please complete the forms and mail along with any noted documentation to the Fund Office.
To change the amount of the taxes withheld from your pension payment, complete a Tax Withholding Election Form and return it to the Fund Office.
In order to change your mailing address, you must complete a Change of Address Form. You may print and complete the form, or you may contact the Fund Office and a form will be mailed to you.
Your surviving spouse or beneficiary may be entitled to a Death Benefit if you pass away prior to receiving benefits from the Plan. The form, amount and commencement date of the benefit payments depend upon various factors, including the date of your death.
If you divorce (whether before or after retirement), your spouse may be entitled to receive a portion or all of your pension benefit in accordance with the terms of a Qualified Domestic Relations Order (QDRO). Under the terms of a QDRO, certain payments could be made from your benefits to pay alimony, child support, or marital property rights to your former spouse, child, or other dependent. If you divorce, you must contact the Fund Office to ensure your benefits are paid properly.

A QDRO may affect the amount of pension benefit you will receive or are receiving. A copy of the Fund’s procedures for handling QDROs will be provided to you free of charge, upon request. If you have questions about QDROs, please contact the Fund Office.
